INSURED PATIENTS:
We will bill your visit directly to your insurance provider if you supply an updated proof of insurance at each visit. Please note that the insurance policy is between you and your insurance provider; we are happy to file the claim for you, but the balance is your responsibility if your plan does not pay after 30 days. Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us.
OUT-OF-POCKET AND UNINSURED PATIENTS:
The full professional fee will be collected at the time of your first and each subsequent visit. Out-of-pocket patients automatically qualify for special consideration in accordance with
our financial assistance program.
MEDICARE:
Currently, we accept patients on the Medicare plan. As a courtesy, we will also bill your secondary insurance provider directly. You will responsible for all costs related to services not covered
under Medicare. On June 25, 2010, Congress passed a 6 month extension on a looming 21+% Medicare physician fee reduction; however, Congress has repeated demonstrated no serious interest in repealing
the "Sustainable Growth Rate" formula or SGR formula which would require a 23% payment reduction in December, 2010. If that is allowed to happen, we will be forced to severely limit Medicare
patients in our practice. If you are interested in preserving Medicare access to our practice, please write your Congressional representatives and make your concerns known.
