In order to register and treat you promptly, we request that our patients bring their current insurance information with them for each visit.

Prior to each office visit, you will be asked to fill out an online questionnaire. This is absolutely mandatory and incredibly important for two reasons. First, the federal government is requiring that all physicians office adopt electronic medical records to avoid potentially dangerous medical errors. Secondly, this process allows us to focus your and our valuable time together on the important problems at hand and not wasting our limited time on gathering information that is irrelevant (but still required) to your current problem(s). Please log onto our Patient Questionnaire website and answer the questions completely and accurately as soon as you arrive in my office. If this is not done, we cannot guarantee that your health won't be compromised and our time together too short.
